Departmental Scholarship Applications
The Department of Mathematical Sciences's Undergraduate and Graduate Scholarship Application forms use conditional logic to ask additional questions where necessary.
The forms' lengths may vary depending on the information provided.

Personal Information
The Department of Mathematical Sciences is now using a virtual, web-app for collecting applications for the Undergraduate and Graduate Department Scholarships.
To start off, you will enter your first and last name separately; an option for a middle name is included, but not required.
Academic Information
Provide University of Arkansas ID number, email, and status as an undergraduate or graduate student applying for the scholarship.
Contact Information
This is where you will provide your primary contact email and an optional phone number we can call if more information is needed. A mailing address will also need to be provided.

Have you graduated from a high school?
If you have graduated from a traditional high school in the United States, we will need the following information:
High School Information
The name and address of the high school. If it is in Arkansas, please also provide which county of Arkansas it's in.
Graduation Information
We'll need to know when you graduated high school and your final GPA.
Some high schools also assign ranks in graduating classes as a percentile, e.g. upper 10%, or as a title, e.g. "Summa Cum Laude" or "Valedictorian." If this does not apply to you, the field is optional.
If you have not graduated from a traditional high school in the United States, we offer other ways of explaining your situation; you may select all that apply.
Home School Students
If you have been home schooled, provide the name and contact information of your primary teacher.
Other
If you wish to provide more information about your situation, you may do so in the text-box provided in the form.

How do you self identify as a student in Mathematical Sciences?
The form will ask if you believe yourself to be part of any minority/underrepresented groups, a non-traditional student, or a first-generation student.
Plans for After-Graduation
If you're planning on graduate school, becoming a teacher yourself, going into the workforce, or are still on the fence, we'd like to know. The application will ask what other plans you have for using your major.

Monetary Awards
Before we award you a scholarship, we will also need information on any other scholarships you have been awarded. Provide the following for each award:
(1) Classification (Scholarship, Fellowship, or Other)
(2) Title of the Award
(3) Date the Award was Issued
(4) Issuing Party
(5) Number of Semesters the Award Covered
Your scholarships and financial awards can also be reviewed in UAConnect; however, be sure to check for any scholarships that may be missing from that list.

Non-Monetary Awards
These are awards you have received that are not financial gifts, but are still marks of notable accomplishments. When asked, provide details of each award including when it was awarded and by whom.
Recommendation
This scholarship applications requires that you have at least one letter of recommendation or informal notice of support from a previous mathematics instructor.
You will need to provide the contact information of each recommender (up to three) and some information about the most recent class you took from each (name of the class, level of mathematics, when you took it, and where you took it).
We have several options available for recommenders to submit their support:
Option 1: Request Virtual Support via Email
This option will use the requester's email address to ask for a notice of support on your behalf from inside the form.
The email will be addressed from ugrdcoor@uark.edu with the subject line being "Request of Notice of Support for Scholarship Applicant {{ Logic - Student Name Literal }}" and you will also get a confirmation when it is sent.
You may include a custom message to be attached at the end of the email to your recommender. This is encouraged to make sure your recommender is able to capture you in the most accurate light based on what they remember of your interactions.
Option 2: Alternate Method of Recommendation
this option may be chosen to bypass the Request Virtual Support system if you already have a sealed letter or recommendation or your recommender is sending their letter of support via mail.

Undergraduate Scholarships and Awards (fix link later)University Program Track
We give graduate applicants the choice choosing between the MATH and STAT program tracks. You will also be asked for the following of each track:
(1) Program Entrance Date
(2) Degree Plan (MS, MA, PhD)
(3) Expected Program Exit Date
(4) First and Last Name of Advisor
(5) Advisor's UARK Email Address
You must also provide a summary of their career plans after graduation.

Curricular, Extra-Curricular, and Community Involvement
Besides the merits and awards asked for above, some scholarships are awarded based on if you are or have ever been involved with any curricular, extra-curricular, or community activities. If so, dates and descriptions of each activity must be provided.
